Grease Trap Monthly Reporting Log

Post this log in a plastic sleeve on the wall in the kitchen in a safe area, protected from food, water, and grease. Please email or fax this form along with all required documents on or before the 28th of each month.

Commercial Sewer Discharge Form

This form is required to be compiled according to our National Pollutant Discharge Elimination System (NPDES) permit. The City is required to maintain a database of all Industrial/Commercial Users who discharge into the sewer system. Please fill out this paperwork and return. For questions, call 574-534-5802.
